Submit a ONE or TWO PAGE concept letter for the intended project, including the following:
- Organization’s legal name and address
- Proof of 501(C)(3) status
- Organization’s Mission
- Brief description of project
- Amount of request and total project cost
- Staff Executive name, phone number and email address
Submit Concept Letter by email to kristin@mcmillenfoundationfortwayne.org, and call 260-444-3927, to leave a voice mail alerting us that your concept letter was sent. Confirmation of receipt will be emailed as soon as possible.
Following review, the Foundation will report back to the organization. If the concept letter is approved, the Foundation will accept a Full Proposal.
